Note: Please note that this feature is available for Expanded Text Ads and Responsive Search Ads only, and you may need to meet some additional requirements. 

  1. Log into your Google Ads account.
  2. In the page menu on the left, click Campaigns.
  3. Click the plus (+) button, and then select New Campaign.
  4. Select Sales Opportunities as your goal.
  5. Select Search as the campaign type.
  6. Select Form Submissions by Potential Customers.
  7. Click Continue.
  8. Choose your campaign settings.
  9. In the 'Ad extensions' list, select Lead form extension.
  10. Create the lead form extension and click Save. From that moment on, this extension will be part of your ad and the message "Always show the form for potential customers when a user interacts with my ad" will be selected underneath.
  11. Keep creating your campaign. When you're done, click Save.
